How this estimator works
This is a general policy heuristic, not a coverage determination. It maps each payer-and-service combination to one of four likelihood bands - Typically covered, Often covered with prior authorization, Sometimes covered (state-dependent), and Rarely covered - based on broad Centers for Medicare & Medicaid Services (CMS) coverage rules and how Medicaid expansion vs. non-expansion states generally treat each service. It does not read your specific plan.
Why state matters: Original Medicare, for example, generally does not cover residential SUD treatment, while Medicaid in expansion states usually covers the full continuum. Coverage is dynamic, always confirm with both the facility intake team and your plan's behavioral-health line before scheduling. According to its National Substance Use and Mental Health Services Survey (N-SUMHSS), the federal record catalogs more than 17,000 substance-use and mental-health treatment facilities nationwide. This directory was last rebuilt from that survey in May 2026, see our methodology for how the data is collected and updated. Always confirm details directly with a facility before relying on them.
